At Breaking Habits, our mission is to restore lives by delivering compassionate, evidence-based behavioral health treatment to individuals navigating substance use and mental health challenges. We are committed to providing accessible, Medicaid-supported care in a safe, structured, and culturally responsive environment—empowering our clients to reclaim independence, reconnect with their communities, and build sustainable pathways to recovery.

More About Breaking Habits

Breaking Habits began with a simple conviction: rural Arizonans deserve the same high-quality behavioral health care that is available in large metropolitan areas. Our founders saw the gap first-hand. Thousands of Medicaid members in western Maricopa, Yuma, and Pinal counties travel hours for treatment or forgo care altogether. By transforming a 64-room hotel in Gila Bend into a 120-bed, AHCCCS-compliant residential and intensive outpatient campus, we are bringing evidence-based services such as detox, MAT, trauma-informed therapy, neurofeedback, and robust weekend programming directly to the communities that need them most.

What sets us apart is our “full census from day one” model. Long before opening, we build referral partnerships with county courts, VA programs, FQHCs, tribal health services, and discharge planners statewide. Every prospective client receives insurance pre-authorization and a confirmed admission date. When our doors open, beds are filled and therapeutic groups are fully staffed. This approach eliminates the slow revenue ramp that hinders many new facilities and allows us to reinvest immediately in staff development, patient amenities, and innovative treatment tracks.

Clinical excellence is anchored by strong compliance practices. Our leadership team includes former ADHS surveyors, veteran Medicaid operators, and a board-certified addiction psychiatrist. We adhere to CARF and Joint Commission standards, implement rigorous quality-improvement cycles, and maintain transparent outcomes reporting. Every staff member, from peer support specialists to nursing supervisors, receives ongoing training in evidence-based modalities and culturally responsive care. This ensures that guests receive treatment that is safe, dignified, and effective.

We also view recovery through a community lens. Clients participate in vocational readiness workshops, financial literacy classes, and wellness activities that continue beyond discharge through our alumni network. By equipping individuals with real-world skills and supportive peer relationships, we help break the cycle of relapse and create ripple effects that strengthen families, workplaces, and neighborhoods across rural Arizona.

From property conversion to long-term aftercare, Breaking Habits is more than a treatment center. We are a catalyst for health equity, economic resilience, and hope in every life we touch.

Garrett Scrivner
Founder/Sole Member (Owner)

A U.S. Army combat veteran turned entrepreneur, Garrett brings a mission-first mindset to behavioral healthcare. After serving as an 11B Infantryman and later leading high-risk executive-protection teams, he shifted his focus to business development, helping early-stage companies secure growth capital and operationalize big visions.

At Breaking Habits, Garrett blends battlefield discipline with startup agility—driving strategy, investor relations, and risk mitigation while empowering a seasoned clinical leadership team to deliver AHCCCS-compliant residential and outpatient care to rural Arizona. His commitment is simple: expand access, uphold top-tier compliance, and build a scalable model that transforms lives and communities alike.

Dr. John Dolores
Chief Executive Officer (CEO)

Dr. John Dolores has a PhD in Clinical Psychology and a law degree (JD) as well as being board certified in Healthcare Administration (FACHE). Dr. Dolores practiced as a Clinical Psychologist for several years before transitioning into administration in 2009. He has led multiple facilities at large companies in the behavioral health space, including Universal Health Services (UHS), Acadia Healthcare, and Summit BHC. 

Dr. Dolores then founded multiple start-ups, one of which he sold to a private equity firm. Following his exit from his company, he then embarked on corporate turnaround projects, joining a couple of medium sized companies as COO to professionalize the business and prepare it for sale. Dr. Dolores is an expert at operations, growth, strategy, development, compliance, technology, human resources, payor relationships, legal, facilities, and credentialing. His leadership style is that of a servant leader with a focus on transparent, collaborative communication and focused mentorship to grow staff to fulfill their potential.

Dr. David R. Hadlock, D.O., FACOOG, DABAM
Medical Director

Dr. David R. Hadlock is a double board-certified physician and nationally recognized leader in addiction medicine and behavioral health. As a Diplomate of the American Board of Addiction Medicine (DABAM) and a Fellow of the American College of Osteopathic Obstetricians and Gynecologists (FACOOG), he brings more than three decades of clinical excellence to his role as Medical Director of Breaking Habits, LLC. 

With specialized expertise in medical detoxification, medication-assisted treatment (MAT), and the management of complex co-occurring disorders, Dr. Hadlock has dedicated his career to delivering high-quality, evidence-based care in both residential and outpatient settings.

Throughout his distinguished career, Dr. Hadlock has served as Medical Director and Clinical Supervisor for multiple residential treatment programs, correctional health facilities, and outpatient addiction clinics. He has held faculty appointments at Idaho College of Osteopathic Medicine, Pacific Northwest University, and Idaho State University, and has provided leadership as past President of the Idaho Society of Addiction Medicine and the Idaho Osteopathic Physicians Association. His extensive experience includes developing and overseeing medical detox protocols, implementing ASAM criteria, and guiding multidisciplinary teams in the treatment of justice-involved and high-acuity populations.

At Breaking Habits, Dr. Hadlock provides strategic medical oversight for the full continuum of care, including medical detoxification, residential treatment, IOP, and MAT services. Mark Tierce
Chief Compliance Officer (CCO)

With more than 15 years of finance and regulatory experience in behavioral healthcare, Mark Tierce ensures that every dollar and every clinical process at Breaking Habits meets the highest standard of fiscal accountability and Title 9/AHCCCS compliance.

A former Big Four healthcare auditor turned operational CFO, Mark has overseen budgets exceeding $100 million, implemented Medicaid billing platforms, and guided multiple facilities through Joint Commission and CARF accreditation. At Breaking Habits, he architects the pro forma, oversees cost-certainty controls, and leads our quality-assurance program -- making sure clinical excellence and financial transparency always move in lockstep.

Kevin “Doc” Ivory, MBA
Chief Operations Officer (COO)

Doc Ivory is an operations executive with advanced training in health administration, business, and psychology, known for a disciplined, people-centered approach to leadership. His work focuses on strengthening operations, refining workflows, and supporting sustainable growth while preserving the human connection essential to effective care.

Doc holds a Bachelor of Science in Health Administration, a Master of Business Administration, and is a doctoral candidate in Psychology. His experience includes senior administrative leadership within the Department of Veterans Affairs, healthcare technology, healthcare insurance account management, and international healthcare operations during the COVID crisis. Combined with advanced FEMA certifications and training in incident command, emergency planning, decision-making, communication, and workplace safety, Doc brings a practical, systems-driven approach to operational challenges to Breaking Habits LLC.
Tiffany Mooney, MA, LPC
Clinical Director

Tiffany Mooney, MA, LPC, is a Licensed Professional Counselor and Behavioral Health Clinic Manager with over a decade of experience providing and overseeing mental health services across diverse clinical settings. Her background includes work in outpatient treatment, corrections, tribal health systems, and school-based services, giving her a broad and culturally responsive perspective in care delivery. Tiffany specializes in assessment, diagnosis, and the development of individualized treatment plans, utilizing evidence-based modalities to support clients across the lifespan. In her current leadership role, she provides clinical supervision and program oversight while continuing to deliver individual and group therapy.

Tiffany is known for her direct, relational supervision style, prioritizing authentic connection with supervisees to better understand their communication styles, strengths, and clinical fit. She emphasizes accountability, skill development, and thoughtful client-therapist matching to improve outcomes and team effectiveness. Her leadership approach balances high expectations with strong support, fostering both professional growth and quality care. Tiffany remains committed to advancing clinical excellence while expanding access to meaningful, client-centered behavioral health services.

Devonte-Miller
De'Vante Miller 
Operation Manager
Originally from Cleveland, Ohio and now residing in Phoenix, Arizona, I am passionate about serving individuals in the behavioral health, mental wellness, and substance abuse rehabilitation space. With a strong foundation in working with SMI populations, I bring a deep commitment to integrity, ethics, and compassionate care. My goal is to enhance the quality of life for every client by creating supportive, empowering environments where recovery and growth are possible. I believe in showing up with purpose and heart, helping people navigate some of the most challenging moments in their lives.
